Safety Incident Report (SIR)

The Safety Incident Report (SIR) provides critical information to assess an incident, injury or near-miss.  Each insight from the affected employee(s) helps Safety Incident Review Team to understand the situation more fully.  The SIR Team has employees from every department and the team reviews each incident at its quarterly meetings, and offers recommendations that helps make Urbandale a safer place to work.
